Office Policies & Payment Information

Office Policies:

As a courtesy, we offer online appointment requests. Please note that once a request has been made, the appointment is not set until we verbally confirm your requested appointment date and time. We will make every attempt possible to accommodate your appointment request. The online appointment request form is not to be used for cancellations or to confirm your appointment. If you have not already done so, you may request an appointment here.

To reschedule or cancel an appointment, you must contact us directly at least forty-eight (48) hours in advance to avoid a missed appointment fee of up to $50. Appointments not kept or changed with less than 48 hours notice are considered missed. Missed appointments prevent others from receiving the dental care they deserve. We take them seriously, so please be considerate, and inform us in advance if you need to change your appointment.

Dental Insurance:

We understand that in today’s economy, finding quality dental care at a cost that fits your budget can be a challenge. Our goal is to remove financial barriers so that all of our patients – regardless of age, income, and dental needs – can receive the care they desire. We are in-network for most PPOs, but even if your plan considers us out of network, call our office about your options. Many insurance companies offer reimbursements according to their payment schedules.

If we are in-network for your dental insurance, please bring your current card and information with you to your first visit. We may ask you to confirm this information at subsequent visits so we can remain up-to-date and fully informed to serve you. We will provide a cost estimate for each procedure broken down by insured and uninsured costs, and we ask that uninsured costs be paid the day services are rendered. As a courtesy, we file all claim forms electronically, provide postage for special claims, and track claims for you.

No Insurance? No problem!

We accept cash, personal checks, and credit/debit cards (Visa, MasterCard and Discover).
We also encourage you to take advantage of Care Credit and Citi Card (18 months no interest). Call our office for more details.
